For people living with complex conditions

Make what's
invisible
visible.

Your records are only half the story. Constellate pulls them into one place, then helps you build the half that was never written down: the stress, sleep, diet, environment, and infections that quietly shape your immune system and today live only in your head. One clear picture your care team can finally act on.

How it works

Holding your whole health history is a mental load you carry alone.
We connect the dots.

Every specialist, portal, and lab result, scattered across systems that don't talk to each other. Constellate gathers it, keeps it current, and connects the dots of your health journey, so you can stop carrying it all in your head.

  1. 01

    Bring in your records

Portals, labs, pharmacies, providers. We pull the medical record from every corner of your care and organize it for you.

  2. 02

Capture what your chart never sees

The stress, sleep, diet, environment, and infections that shape your immune system but were never written down. We help you get them out of your head and into your history.

  3. 03

See it all as one living picture

Your records and your lived context in a single timeline, so you and your care team can read the whole story, not just the visits.

What you get

Four things every complex-condition patient deserves, all in one place.

Records

One record, finally whole.

We pull records from your providers, hospitals, labs and pharmacies. Then we de-duplicate, timeline, and translate them into something you can actually read.

Care assistant

A companion between visits.

Your immune system is shaped by more than your chart. Between visits, your assistant helps you capture the rest: stress, sleep, diet, a new environment, an infection. Then it answers questions and preps you for your next appointment.

My knee's been worse this week—anything I should tell Dr. Alvarez?
Since your last visit: 3 flare days, methotrexate refilled on time, CRP was 8.4 (up from 5.1). I've drafted a visit summary you can review.

Research

Research that keeps up with you.

New studies, guidelines and treatment options for your specific condition—surfaced when they matter, in language you don’t need a PhD to parse.

What we believe

Empowerment over your health isn't a feature. It's the point.

  1. 01

    Your records belong to you.

Not to a hospital system, not to us. We give you access, portability, and the right to walk away with everything.

  2. 02

Software should not replace your doctor.

It should bring you into the room as an equal—informed, organized, and heard. Care is still a human relationship.

  3. 03

Complex conditions deserve a coherent picture.

Fragmented care isn't a personal failing. It's a design problem. We're fixing it, one condition at a time.

Where we're starting

Starting with autoimmune. Built for the conditions the system keeps overlooking.

These are the conditions we're focused on today. If yours isn't here yet, join the waitlist—we're onboarding new communities in cohorts.

Questions

Answered
honestly.

Does Constellate replace my doctor?

No—and that’s the point. Constellate brings you and your care team into alignment so appointments start from a shared picture instead of from scratch. Your providers stay in charge of your care.

Is Constellate a medical device or does it diagnose?

No. Constellate is not a medical device and does not diagnose, treat, or replace medical advice. We consolidate your records, help you understand them, and prepare you for conversations with your clinicians.

Who owns my data?

You do, always. You choose which providers and caregivers can view your record, and you can export or revoke access at any time. Constellate is HIPAA-aligned and never sells personal health data.

What conditions do you support today?

We’re starting with autoimmune conditions—lupus, RA, psoriatic arthritis, Sjögren’s, Hashimoto’s and more—and expanding to adjacent complex conditions in the months ahead.

When does the care advocate program open?

Care advocates are in private beta with a small cohort. Join the waitlist and we’ll reach out when spots open up in your region.

Your records miss everything that happens between visits.

The stressful month, the bad stretch of sleep, the flare that faded before your appointment. Constellate captures the life your chart never sees, so you walk into every visit with the whole picture. Early cohorts are small and hands-on.
